Free Counseling Award Application

We provide free counseling awards to families in need of financial assistance for their child(ren).  If you are interested, please fill out the form below and email it to laloboyfoundation@gmail.com.  

If you have any questions, call our office at 480-612-4656. 

Monday through Friday 9:00 a.m. to 5:00 p.m.

 We advocate for and spread awareness about mental health in Arizona, 

and provide financial assistance for low-income youth in need of counseling support.
